| Medication Consent Form |
| A completed medication consent form needs to be on file at school in order for office staff to be able to give your child medication. If it is a prescription medication, a doctor's signature is required. All medication must be sent to school in it's original container. |

| Student Planned Absence Form |
| If your child is going to be gone for several days, please fill out one of these forms and return to the school office. The classroom teacher will put together homework for the days that your child will miss school. |

| Behavior Plan Signature Page |
| This form will also be kept on file in the school office. All district elementary schools follow this plan. Once signed, this page will be kept until your child enters 6th grade. |
